fa الگوی اپیدمیولوژیک تب مالت در ایران طی سال ۱۴۰۱: تحلیل توصیفی بر اساس داده‌های ملی پایش بیماری Epidemiological Pattern of Human Brucellosis in Iran During 2022/23: A Descriptive Analysis of the National Surveillance Dataset میکروب شناسی پزشکی Medical Microbiology پژوهشی اصیل Original Article تب مالت (بروسلوز) یکی از مهم‌ترین بیماریهای زئونوز بومی ایران است که عمدتاً از طریق تماس با دام آلوده و مصرف فرآورده‌های لبنی غیرپاستوریزه به انسان منتقل می‌شود. هدف این مطالعه، توصیف الگوی زمانی–مکانی و ویژگی‌های جمعیت‌شناختی و شغلی موارد انسانی تب مالت در ایران طی سال ۱۴۰۱ است. مطالعه حاضر توصیفی–مقطعی و بر اساس مجموعه‌داده ملی پایش تب مالت (سال ۱۴۰۱) انجام شده است. متغیرها شامل استان محل سکونت، ماه بروز، منطقه جغرافیایی (شهری/روستایی/حاشیه/عشایری/سیاری)، جنس، گروه سنی و شغل بودند و به صورت شمارش و درصد محاسبه شد. در سال ۱۴۰۱ مجموعاً ۲۲۰۷۳ مورد تب مالت انسانی گزارش شد. پنج استان با بیشترین شمار موارد عبارت بودند از خراسان رضوی (۴۰۳۴؛ 3/18 درصد)، همدان (۱۹۴۱؛ 8/8درصد)، آذربایجان غربی (۱۹۰۳؛ 6/8 درصد)، لرستان (۱۷۴۳؛ 9/7 درصد) و آذربایجان شرقی (۱۴۷۹؛ 7/6 درصد) که در مجموع 3/50 درصد همه موارد کشور را تشکیل دادند. مردان 7/57 درصد و زنان 3/42درصد مبتلایان را شامل شدند، 1/71 درصد موارد از مناطق روستایی گزارش و توزیع سنی نشان داد گروه‌های ۳۰–۳۹ سال (3/22 درصد) و ۴۰–۴۹ سال (6/20 درصد) بیشترین سهم را دارند. از نظر شغلی، خانه‌دار (8/29 درصد) و دامدار (8/ 27درصد) غالب بودند. پیک ماهانه در خرداد (12درصد) و تیر (1/12درصد) مشاهده شد؛ تابستان 33درصد و بهار 8/28درصد موارد را دربرگرفت. الگوی بیماری در سال ۱۴۰۱ نشان‌دهنده میزان شیوع بالای بیماری در چند استان غربی–شمال‌غربی و خراسان رضوی و غالب بودن مناطق روستایی نسبت به شهری و پراکندگی آن در بین گروه‌های سنی میانسال و نیز نقش مهم مواجهات شغلی‌/خانگی مرتبط با دام و لبنیات سنتی می باشد. تقویت راهبردهای «سلامت واحد یا One Health» شامل واکسیناسیون هدفمند نشخوارکنندگان کوچک در استان‌های پربار، بهداشت لبنیات و ایمنی شغلی، و مداخلات آموزشی پیش از فصل اوج می‌تواند بار بیماری را کاهش دهد. Brucellosis remains endemic in Iran, primarily transmitted to humans through contact with infected livestock and consumption of unpasteurized dairy products. This study describes the spatiotemporal and demographic/occupational profile of human brucellosis cases in Iran during Year March 2022–March 2023. We conducted a descriptive cross-sectional analysis of Iran’s national human brucellosis surveillance dataset from March 2022–March 2023. Variables included province, month/season, residential setting (urban/rural/peri-urban/nomadic/mobile), sex, age group, and occupation. Results are presented as counts and proportions. A total of 22073 cases were reported. Five provinces—Razavi Khorasan (4,034; 18.3%), Hamadan (1,941; 8.8%), West Azerbaijan (1,903; 8.6%), Lorestan (1,743; 7.9%), and East Azerbaijan (1,479; 6.7%)—accounted for 50.3% of all cases. Males represented 57.7% and females 42.3% (male-to-female ratio ≈1.36). Rural areas contributed 71.1% of cases. Middle-aged groups 30–39 years (22.3%) and 40–49 years (20.6%) predominated. By occupation, housewives (29.8%) and livestock breeders (27.8%) were most affected. Seasonal peaks occurred in June (12.0%) and July (12.1%); summer accounted for 33.0% and spring for 28.8% of cases.
