fa بررسی خصوصیات اپیدمیولوژیک و روند بروز بیماری تب مالت در شهرستان کمیجان طی سال‌های 1390 تا 1398 Investigating the epidemiological characteristics and incidence trends of Brucellosis in the Komijan city during the years 2011 to 2019 بهداشت عمومی Public health پژوهشی اصیل Original Article تب مالت به عنوان یکی از مهم‌ترین بیماری‌های مشترک انسان و دام محسوب می‌گردد. این بیماری به علت ایجاد سقط <a href="https://fa.wikipedia.org/wiki/%D8%AC%D9%86%DB%8C%D9%86" title="جنین">جنین</a> در دام، کاهش تولید <a href="https://fa.wikipedia.org/wiki/%D8%B4%DB%8C%D8%B1_(%D8%AE%D9%88%D8%B1%D8%A7%DA%A9%DB%8C)" title="شیر (خوراکی)">شیر</a>، <a href="https://fa.wikipedia.org/wiki/%D8%B9%D9%82%DB%8C%D9%85%DB%8C" title="عقیمی">عقیمی</a> دام‌های مبتلا و همچنین به علت ابتلای انسان به بیماری تب مالت، همواره از دو بعد اقتصادی و <a href="https://fa.wikipedia.org/wiki/%D8%A8%D9%87%D8%AF%D8%A7%D8%B4%D8%AA" title="بهداشت">بهداشتی</a> مورد توجه قرار می‌گیرد. این مطالعه با هدف بررسی خصوصیات اپیدمیولوژیک و روند بروز بیماری تب مالت در شهرستان کمیجان انجام شده است. در این مطالعه مقطعی، اطلاعات دموگرافیک و اپیدمیولوژیک 379 فرد مبتلا به تبمالت که در فاصله سالهای 1390 تا 1398 به مراکز بهداشتی و مطبهای شهرستان کمیجان مراجعه کرده بودند، مورد بررسی قرار گرفت. برای تجزیه و تحلیل از مدلهای آماری توزیع فراوانی، آزمون تیمستقل، کایدو تک متغیره و رگرسیون لوجستیک با استفاده از نرم افزار SPSS نسخه 25 استفاده شد. سطح معنی داری آزمونها 05/0 در نظر گرفته شد. بروز بیماری در فاصله سالهای 1390 تا 1398، 98 درصدهزار نفر بود. میانگین سنی مبتلایان 97/18±20/38 سال بود و بالاترین فراوانی موارد گزارش شده در گروه سنی 25 الی 44 سال با 38درصد قرار داشت. میزان ابتلا در مردان(67 درصد) بالاتر از زنان(33 درصد) بود. از نظر شغلی بیشترین بروز بیماری در دامداران و کشاورزان با 9/37درصد بود. 2/70 درصد از بیماران تب مالت ساکن روستا و فقط 8/29 درصد ساکن شهر بودند که این اختلاف از لحاظ آماری معنیدار بود(001/0>p). نتایج مطالعه نشان داد که بیماری در بین مردان دامدار وکشاورز بیشتر بوده و از الگوی فصلی مشخصی برخودار است لذا توجه خاص به این گروه پرخطر ضروری به نظر میرسد. با توجه به مشکلات اقتصادی و اجتماعی فراوان ناشی از بیماری لازم است تب مالت در اولویتهای بهداشتی قرار گرفته و آموزشهای لازم در مورد راههای انتقال بیماری به گروههای مختلف جمعیت بخصوص افراد در معرض خطر داده شود.   Brucellosis is considered one of the most important diseases of humans and animals. Due to abortion, reduced milk production, infertility of infected animals, and transfer to humans, this disease is always considered from two economic and health aspects. This study was conducted to investigate the epidemiological characteristics and the incidence of Brucellosis in Kamijan city. This cross-sectional study examined demographic and epidemiological information of 379 people with Brucellosis who had referred to health centers and offices in Komijan between 1390 and 1398. Frequency distribution statistical models, independent t-test, univariate chi-square, and logistic regression were used for analysis using SPSS software version 25. The significance level of the tests was considered 0.05. The disease incidence between 1390 and 1398 was 98 per 100,000 population. The mean age of patients was 38.20±18.97 years, and the highest frequency of reported cases was in the age group of 25-44 years with 38 percent. The infection rate was higher in men (67 percent) than in women (33 percent). Occupationally, the highest disease incidence was in ranchers and farmers, with 37.9 percent. A 70.2 percent of patients lived in rural areas, and only 29.8 percent lived in urban areas; this difference was statistically significant (p<0.001). This study showed that the disease is more common among rancher-farmer men and has a specific seasonal pattern, so special attention to the high-risk group is essential.
